All patients will have a phone screening prior to their appointment

Patient forms will be filled out at home, as instructed

Strict social distancing will be upheld in our waiting room

All patients must wear their own masks

Friends and family must wait outside, unless with a minor

Temperature will be checked and hands sanitized upon arrival
